Emergency Release Form
Here is a release form that every participant, including adults, must fill out prior to coming on the trip. The form must be notarized and turned in to your trip coordinator at your local church. The Youth Director or trip coordinator will bring the original notarized forms for every participant to turn in upon arrival.

This form will be filled out online by the Youth Director or trip coordinator by February 1 for all spring break mission trips and by May 1 for all summer mission trips.
Safe Environment Verification
The Youth Director or trip director will have the pastor or principal verify that all participants 18 or older at the time of the mission trip are in full compliance with diocesan safe environment policies. A letter should be scanned and emailed to Michelle Perez at michelle@catholicmissiontrips.net or mailed to Catholic Mission Trips, Inc. at 43508 Postrail Sq. Ashburn, VA 20147 by February 1 for spring break mission trips and by May 1 for summer mission trips.
