Release Form
Here is a release form that every participant, including adults, must fill out prior to coming on the trip.  The form must be notarized and turned into your trip coordinator at your local church.  The Youth Director or trip coordinator will mail in the original notarized forms for every participant no later than 2 weeks before the trip.
